Governors Order 13D
On Friday (September 17th, 2021), the Governor provided updates and clarification regarding proof of vaccination and the testing requirements for staff who are not vaccinated. For staff that have been vaccinated, an additional authenticity of vaccination is required. That form is attached below. We ask that you please print, sign and submit the Authenticity form by end of the day on Wednesday, September 22nd to the same individual you provided your proof of vaccination record to.
For staff who have not yet handed in proof of vaccination, we ask that you please provide a copy of your vaccination records and your authenticity of vaccination form, by 4PM on Wednesday, September 22nd, 2021
Staff who have not been vaccinated, or have not provided proof of vaccinations, will be required to submit Covid 19 test results by Monday morning (September 27th, 2021) prior to the start of the workday. We ask that you submit those results to your building administrator. Your test result must state that you have a "negative" or "not detected" result. Per the Governor's Order, staff who have not provided proof of vaccination and do not provide the appropriate test result documentation, will not be able to report to work until they have that documentation. Additionally:
Those employees must submit to testing every 7 days and submit the results within 72 hours of administration of the test. Test results will be required every Monday prior to the start of the school day. In addition, to comply with the testing requirement, testing must be
- either PCR or antigen SARS-CoV-2, and
- administered and reported by a state-licensed clinical laboratory, pharmacy-based testing provider, or other healthcare provider facility with a current Clinical Laboratory Improvement Amendments waiver.
Of note, results provided as “inconclusive” are not considered negative results and as such require retesting. If an individual receives an inconclusive result and cannot be retested and provide a negative result within 7 days of their last negative test, then that individual may not report to work until they can provide a negative test result. Home-based testing and results obtained outside of a facility of the type indicated above are not considered adequate proof.

Temporary testing waivers for staff who recently had Covid 19.
Staff who wish to request a temporary waiver from SARS-CoV-2 testing on the basis of having had COVID-19 within the prior 90 days must submit a copy of a temporary waiver request from the Personnel Office. The form must be completed and signed by their healthcare provider and provided to your school prior to the start of the school day on September 27th, 2021. Any individual granted a temporary waiver from SARS-CoV-2 must return to regular weekly testing after the expiration date indicated on the waiver form if they are not fully vaccinated by that date.
